Which forms should I complete before my first visit?

Please complete the Patient Registration and Medical History forms before your first visit. These forms help our team review your contact information, insurance details, medical history, medications, and treatment concerns before your appointment.

Can I download post-op instructions after my procedure?

Yes. The Patient Center includes post-op instructions for periodontal and implant surgery, Pro Arch treatment, sinus procedures, scaling and root planing, extractions, and Arestin therapy. Some instructions are also available in Spanish.

What should I bring to my first appointment?

Bring a photo ID, insurance card, completed patient forms if available, a list of medications and supplements, recent dental X-rays if your general dentist has them, and any questions you want to discuss with the specialist.

Do I need recent dental X-rays?

Recent dental X-rays are helpful, but they are not required to request an appointment. If additional imaging is needed, our team will explain what is recommended during your visit.
